Forms
Students who wish to change their schedules can download these forms.
Undergraduate students in the School of Education must have advisor's
signature and Dean's stamp in order to present these forms to the
Enrollment Center. Graduate students do not need Dean's stamp, but
some programs require an advisor's signature.
Application for Change of School (PDF)
This form is required
of undergraduate students wishing to change schools within KU. Change
of School forms are included with applications to SOE; once a student
is formally admitted the form will be sent to the Admissions Office
for processing.
Course
Repeat Form
Students shall be allowed to repeat for credit a course in which
they have already received a grade of D or F without the consent
of the dean of the College or School in which the course is offered.
A student retaking a course in which he or she originally received
the grade of D or F may elect to replace the original grade with
the new grade, subject to the restrictions in 2.2.10. Students seeking
to replace a grade of D or F in a semester-long course originally
taken at the University of Kansas must register their choice in
the dean's office of the school where the course is offered no later
than the fifth and sixth weeks of the semester. In all eight-week
courses, including summer session, students must register for course-retake
no later than the third week of class. In courses of other duration,
students must register their choice no later than the week that
40 percent of the class sessions have been completed. In courses
shorter than one week, students must register their choice before
the first class session. The original grade earned by a student
will appear on the student's transcript, but it will not be calculated
into the total hours earned. This rule does not apply to those courses
for which it is assumed new work occurs during the repeated enrollment
under the original title, such as independent study, physical education
skill instruction courses, and thesis or dissertation hours.
Graduation
Check (PDF)
This form is to be used by undergraduate students in the School
of Education (or their advisors). Students' cumulative and major
grade point average, jr/sr hrs, Education hours and total hours
are listed, and calculations are made to determine that they will
have enough hours to graduate. In addition, the courses still needed
to graduate are listed. The graduation check is a tool to be used
by students and their advisors. Errors on this form do not exempt
students from graduation requirements. Students should go to Room
208 JRP to initiate the request and to pick up the results.

Universal
Petition (PDF)
This form is to be used by both graduate and undergraduate students
in the School of Education for course substitution requests, exception
to number of hours taken, non-residency during final 30 hours, time
extension requests, leaves-of absence from a program, to request
an exception to a specific program requirement, etc Upon completion
students should submit this form to their advisor. The typical time
frame for resolution of a petition is 1-2 weeks. Students are responsible
for submitting a complete petition and, with their advisor, monitoring
its progress through the system. Students will be notified by email
of the petition results. This petition form is effective with the
spring 2003 semester.

Transcript
Request
You can order transcripts in person at the windows in Strong Hall
or by mail. If you are a current or former KUMC student, you must
order transcripts from the Office of the Registrar on the KUMC campus.
If ordering in person, you will not be able to take it with you
the same day. If ordering in person, you will need a photo ID and
$10 per transcript. The windows in Strong Hall are open 8-5 weekdays.
The Office of the Registrar at KUMC is open 8-4:30 weekdays.
Transcript
Evaluation
Transcript analyses are performed to determine a person's status
for certification in the state of Kansas. Students wishing to pursue
the Graduate Licensure Program (secondary Math, Science or Foreign
Language) or working teachers wishing to add endorsements may utilize
this option. For students wishing to pursue an undergraduate program
for certification, an analysis will be performed by the Office of
Admissions and Scholarships when you apply to KU.

Students must complete and submit an application before the deadline. See note below about Change of School forms.
Tentative Evaluation of Transfer Credit (PDF)
This form is required of undergraduate students pursuing a degree
in Education who wish to take a course outside of the University.
Students should call Belinda Reynolds at 864-9606 to make an appointment
to discuss transferring the course to KU.
Application for Degree - This form is required for all students who wish to graduate. Students need to complete and submit this form to the School of Education, who will forward it to the Registrar's Office. If the student does not graduate in the term specified, they must complete a new form for the semester in which they wish to graduate. Deadlines for submission are:
